  9. 1.  The application is meant to be filled out using a text editor (such as
  10.     Windows Notepad or DOS equivilent).  If you'd like to fill it out by
  11.     hand just add carriage returns and lines if you wish.

  13. 2.  We're currently looking into accepting credit card payments but not yet!
  14.     At present this application has to be printed out and mailed via US Post
  15.     with a check or money order.  Sorry for the inconvenience.

  17. 3.  If you are using a proportional font Window or OS/2 program for CIS mail,
  18.     you may want to select "Fixed Font" for this window.  The application
  19.     will be a bit easier to read.

  21. 4.  The phone number currently listed at the bottom of this document is mine.
